Pharmacological properties

Pharmacodynamics. benzocaine reversibly stabilizes the neuron membrane, which reduces its permeability to sodium ions. depolarization of the neuron membrane is inhibited, and as a result, the generation and conduction of nerve impulses is blocked. thus, benzocaine has a pronounced local analgesic effect.

Pharmacokinetics. Drug for local use. Benzocaine, which is part of it, is excreted in mucus or, in extremely small amounts, by the kidneys.

Indication

Relief advance, ointment: external and internal hemorrhoids, erosions, cracks, diseases of the rectum, accompanied by severe pain syndrome, itching, burning in the anorectal region; in the postoperative period in patients who underwent proctological operations; for pain relief during diagnostic manipulations.

Relief Advance, suppositories: external and internal hemorrhoids, cracks, erosion and microtraumas in the anus area, perianal itching with pronounced pain syndrome. As an anesthetic after proctological surgical interventions.

Application

Ointment. Adults and children over 12 years of age should use the drug after previous hygiene procedures. remove the protective cap from the applicator. attach the applicator to the tube and squeeze out a small amount of ointment to lubricate the applicator. the ointment is carefully applied through the applicator to the affected areas outside or inside the anus up to 4 times a day, especially in the evening, in the morning or after each bowel movement for 7-10 days. after each use, thoroughly wash the applicator and place it in the protective cap. do not use if the protective coating is missing or damaged.

Suppositories. The drug should be used after performing hygienic procedures. Insert 1 suppository into the anus in the morning, after each bowel movement and before going to bed. Daily dose – no more than 4 times a day. Regular use of the drug allows you to ensure a stable therapeutic effect, relieves the symptoms of hemorrhoids. The duration of treatment is usually 1 week. If necessary, it can be extended up to 3 weeks or more (after consultation with a doctor).

Children. The effectiveness and safety of the drug Relief Advance in children under 12 years of age has not been studied. There are no data. For children under 12 years of age, Relief Advance suppositories are prescribed by a doctor only in case of extreme necessity.

Contraindication

Increased individual sensitivity to any component of the drug and other amide local anesthetic agents, thromboembolic disease, granulocytopenia.

Side effects

From the side of the blood and lymphatic system: when using the drug, methemoglobinemia is possible (cyanosis of the skin, lips and nail beds, headache, dizziness, shortness of breath, weakness, tachycardia).

General disorders and changes at the site of application: allergic reactions, including hyperemia, skin rash, itching, changes at the injection site (irritation, swelling, pain).

From the side of the skin and subcutaneous tissue: allergic dermatitis, contact dermatitis.

Excipients methyl parahydroxybenzoate and propyl parahydroxybenzoate can cause allergic reactions (possibly delayed).

Special instructions

In case of profuse bloody secretions from the anus or in the presence of symptoms of the disease within 7 days of treatment, or in case of deterioration of the condition, it is necessary to consult a proctologist.

You should consult a doctor before starting the use of Relief Advance suppositories, and use Relief Advance ointment with caution as prescribed by a doctor in the following cases: severe hypertension, severe heart rhythm disorders, clinically pronounced thyrotoxicosis, urination disorders.

It is necessary to use the minimum necessary number of Relief Advance suppositories. In the absence of other doctor’s instructions, do not exceed the recommended daily dose of Relief ® Advance ointment to minimize systemic adverse reactions. Relief Advance ointment is not recommended to be applied to damaged skin, it may promote systemic absorption. In case of bleeding from the anus, it is necessary to consult a doctor as soon as possible.

There are reports that the appointment of products containing benzocaine can cause methemoglobinemia. Symptoms such as cyanosis of the skin, lips and nail beds, headache, dizziness, shortness of breath (difficulty breathing), weakness, tachycardia noted during treatment may indicate methemoglobinemia, which is potentially life-threatening and requires immediate medical intervention.

Propylene glycol can cause skin irritation. Methylparaben and propylparaben can cause allergic reactions (possibly delayed).

Use during pregnancy and breastfeeding. During pregnancy, the drug should be used only in case of extreme necessity, after a careful assessment by the doctor of the ratio benefit for the mother / risk for the fetus or child.

During the period of breastfeeding, a decision should be made to stop breastfeeding or refuse to use the drug, taking into account the importance of taking the drug for the mother.

The ability to influence the speed of reaction when driving a motor vehicle or working with other mechanisms. Relief Advance, suppositories: unknown. Relief Advance, ointment: no effect on the ability to drive vehicles or work with other mechanisms was observed.

Interactions

It is not recommended without strict medical indications and doctor’s supervision to use the drug against the background of regular intake of MAO inhibitors, antidepressants and hypotensive agents due to the theoretical possibility of reducing the severity of the effect of MAO inhibitors or increasing the effect of hypotensive agents as a result of interaction with benzocaine in cases of significantly exceeding the recommended doses and duration of administration.

Overdose

Overdose symptoms may be related to the pharmacological effects of benzocaine. its systemic absorption in case of an overdose may manifest itself as drowsiness, anxiety, heart rhythm disturbances, disturbances. in severe cases, convulsions, coma, decreased respiratory rate or respiratory failure may occur. there are reports that prescribing products containing benzocaine can cause methemoglobinemia. symptoms such as cyanosis of the skin, lips and nail beds, headache, dizziness, shortness of breath (difficulty breathing), weakness, tachycardia, which may be observed during treatment, may indicate methemoglobinemia, which is potentially life-threatening and requires urgent medical intervention.

Emergency aid in case of overdose and the development of methemoglobinemia consists in intravenous administration of methylene blue. Other symptoms of overdose require withdrawal of the drug and symptomatic treatment.

Storage conditions

At a temperature not higher than 25 °C. opening the tube and starting to use the ointment does not affect the shelf life, provided that it is tightly closed with a cap every time after use. do not use suppositories if the protective plastic cover is missing or damaged.
