Pharmacological properties
Broad spectrum anthelmintic.
Pharmacodynamics: Pyrantel pamoate is an anticholinesterase agent.
The depolarizing effect of pyrantel disrupts synaptic transmission in the body of helminths (first it enhances, then inhibits), leads to the development of neuromuscular blockade and, as a result, to spastic paralysis of the muscles of helminths, contributing to their excretion with feces. It acts on both sexually mature and immature individuals of helminths sensitive to the drug, does not act on larvae in the migration stage.
Pharmacokinetics. It is practically not absorbed in the digestive tract (3.5%). After a single dose of the drug at a dose of 10 mg/kg of body weight, the concentration of Pyrantel in the blood plasma is insignificant – 0.05-0.13 mg/ml for 1-3 hours. After absorption into the blood system, it is partially metabolized in the liver. The main metabolite is N-methyl-1-3-propanediamine, about 1.2-4.6% of which is excreted in the urine. More than 93% of pyrantel is excreted unchanged in the feces.
Indication
Treatment of helminthiasis: ascariasis, enterobiasis, hookworm, necatoriasis, trichostrongylosis.
Application
It is prescribed orally in 1 dose at any time of the day; there is no need to abstain from eating or take laxatives before using pyrantel.
For enterobiasis and a combination of enterobiasis with ascariasis, the drug is prescribed to adults and children at a single dose of 10 mg/kg of body weight or according to the following scheme:

In severe forms of the disease, a double dose is prescribed – 20 mg/kg/day in 1-2 doses for 2-3 days.
To prevent autoinvasion, repeated administration of pyrantel after 3 weeks at a dose of 10 mg/kg of body weight is recommended.
For isolated ascariasis – 5 mg/kg/day in one dose.
For hookworm, necatoriasis, a combination of necatoriasis with ascariasis, or other combined helminthic invasions, the drug is used at a dose of 10 mg/kg/day for 3 days or 20 mg/kg/day for 2 days.
Children under 3 years of age are prescribed the drug in the form of a suspension for oral use.
Contraindication
Hypersensitivity to the drug; hepatic insufficiency.
Side effects
The drug is usually well tolerated.
The following side effects are possible:
Gastrointestinal: epigastric pain, nausea, vomiting, diarrhea, anorexia; abdominal cramps, tenesmus.
On the part of the hepatobiliary system: increased activity of hepatic transaminases.
Neurological disorders: headache, dizziness, drowsiness, fatigue, insomnia, weakness.
Skin and subcutaneous tissue disorders: skin rash, itching, urticaria.
Special instructions
No special preparation or laxatives are required before using pyrantel.
The tablets must be chewed thoroughly before swallowing.
In case of enterobiasis, all people living together are treated simultaneously.
To achieve an effect in the treatment of enterobiasis, strict adherence to personal hygiene rules is necessary, especially for children, as well as repeated administration of pyrantel after 3 weeks.
After completing the course of treatment, a control stool test is performed for the presence of helminth eggs.
Pregnancy and breastfeeding. Experimental studies have not revealed the teratogenic effect of Pirantel. During pregnancy and breastfeeding, the drug is used after a careful assessment of the benefit-risk ratio for the woman, which is determined by the doctor, and the recommended doses are strictly adhered to.
Children. For children under 6 years of age, it is more appropriate to prescribe the drug in the form of a suspension. For children over 3 years of age, if it is impossible to accurately determine the dose or if swallowing is difficult, the drug is prescribed in the form of a suspension for oral administration.
Ability to influence the reaction speed when driving vehicles or other mechanisms. If dizziness, drowsiness, weakness occur during the use of the drug, you should refrain from driving vehicles or other mechanisms.
Interactions
When pyrantel is used simultaneously with levamisole, the toxicity of the latter increases; the drug should not be used together with levamisole.
Pyrantel should not be used together with piperazine, which has an antagonistic effect on Pyrantel.
Overdose
When taking the drug in very high doses, its side effects, ataxia, may be increased.
In case of overdose – gastric lavage, monitoring of vital body functions, symptomatic therapy.
If there are symptoms of anticholinesterase action, atropine is administered.
