Pharmacological properties

Pharmacodynamics. Zalain / Zalain ovuli is an antifungal drug, an imidazole derivative, with high fungicidal activity, intended for topical use in gynecology. The mechanism of action is to inhibit the synthesis of ergosterol and increase the permeability of the cell membrane, which leads to the destruction of pathogens. Effective against pathogenic yeast fungi (candida albicans, candida spp. and malassezia furfur), dermatophytes (trichophyton, epidermophyton and microsporium spp.) and pathogens that cause infectious diseases of the skin and mucous membranes, including gram-positive strains (staphylococcus, streptococcus).

Pharmacokinetics: There is no systemic absorption.

Indication

Zalain cream. Local treatment of fungal skin infections caused by dermatophytes and yeast fungi: athlete’s foot, inguinal athlete’s foot, smooth skin dermatomycosis, trichophytosis of the beard and mustache area, dermatomycosis of the hands, candidiasis, lichen planus.

Zalain ovules, vaginal suppositories. Local treatment of vaginal candidiasis.

Application

Cream. The cream is applied 1 or 2 times a day (preferably at night or in the morning and evening) in a thin layer gently and evenly to the affected area of ​​the skin, capturing approximately 1 cm of healthy skin around it.

The duration of treatment depends on the etiology of the pathogen and the location of the infection. As a rule, use for about 4 weeks is recommended to achieve clinical remission with a negative microbiological control study to prevent relapse, but in many cases clinical recovery occurs earlier – between the 2nd and 4th weeks.

Pessaries. Adult women: 1 pessary is inserted deep into the vagina in the evening before bedtime once a day. If clinical signs of the disease do not disappear, the drug can be repeated after 7 days.

Contraindication

Cream: history of hypersensitivity to any antifungal derivatives of the imidazole group or to any other component of the drug.

Pessaries: hypersensitivity to antifungal drugs, imidazole derivatives or any of the excipients of the drug; simultaneous use of the drug with latex condoms or pessaries is contraindicated.

Side effects

Cream. The drug showed good tolerability when applied to both intact and damaged skin.

Local contact dermatitis (dry skin, burning, itching, urticaria) may occur. Hypersensitivity reactions, local transient erythematous reactions that do not require discontinuation of the drug are possible.

The medicine contains methyl parahydroxybenzoate, which may cause allergic reactions (possibly delayed).

The drug contains sorbic acid, which can cause local skin reactions (e.g. contact dermatitis).

Pessaries. Sometimes a transient local irritation reaction (burning sensation and itching) may occur. This may be a consequence of the action of cell fragments during the death of pathogens, so such a reaction is most likely a sign of successful therapy. Allergic reactions often occur.

Special instructions

During treatment, it is not recommended to use soap with an acidic pH, you should use mainly cotton underwear, and do not douche.

It is also advisable to apply Zalain antifungal cream to the vulva and perineum area.

When using the drug, you should refrain from sexual intercourse. It is recommended to consider the possibility of simultaneous treatment of the sexual partner.

Therapy can be carried out during menstruation.

Factors (hygienic or lifestyle) that contribute to the development and manifestations of the fungal infection should be identified and eliminated. It is also recommended to treat other pathogenic microorganisms that may be associated with candidiasis.

Therapy should be discontinued in the event of a local allergic reaction.

In the absence of characteristic clinical signs of vaginal candidiasis, a positive microbiological test result alone is not an indication for treatment.

Zalain cream should not be used in ophthalmology. Patients should be cautious when using acidic detergents, as the growth of Candida spp. increases in an acidic environment.

Use during pregnancy or breastfeeding. There is no data on the presence of embryotoxicity and teratogenic effects of sertaconazole. Given the method of administration (single dose treatment) and the lack of systemic absorption, sertaconazole can be used in pregnant women only in cases where the expected benefit to the mother outweighs the potential risk to the fetus.

There is also no data on the penetration of sertoconazole into breast milk. Therefore, the drug should not be used during breastfeeding, except in cases where, in the opinion of the doctor, the expected benefit to the mother outweighs the potential risk to the child.

After external application of the drug in large quantities, plasma levels were not observed. Since the harmful effects of the drug on newborns or pregnant women have not been studied, before prescribing Zalain during pregnancy and breastfeeding, the benefit-risk ratio for the woman and the fetus/child should be assessed.

Children. Do not use in children.

The ability to influence the reaction speed when driving vehicles or working with other mechanisms. Does not affect.

Interactions

For the drug in the form of a cream, interactions have not been established. When using pessaries simultaneously with local contraceptives, a decrease in their spermicidal effect may be observed. It is not recommended to use it together with latex condoms or pessaries due to the risk of their damage.

Overdose

When using the cream, overdose is unlikely. After vaginal use of pessaries, overdose was practically not observed.

Storage conditions

Cream: at a temperature not exceeding 25 °C. Pessaries: at a temperature not exceeding 30 °C.
