Pharmacodynamics. silymarin is a herbal active component of the drug Carsil Forte, obtained from the extract of the fruits of the milk thistle plant (silybum marianum).

The drug has a hepatoprotective and antitoxic effect. Carsil Forte inhibits the penetration of toxins into liver cells (shown for the poisons of the pale toadstool), and also causes physical and chemical stabilization of the cell membrane of hepatocytes.

The antioxidant effect of the drug Carsil Forte is due to the interaction of silibinin with free radicals in the liver and their transformation into less toxic compounds. Thus, the LPO process is interrupted and further destruction of cellular structures does not occur; toxins are neutralized physiologically.

Carsil Forte stimulates the biosynthesis of structural and functional proteins and phospholipids (due to the specific stimulation of RNA polymerase A) and accelerates the regeneration of liver cells.

The clinical effect of the drug Carsil Forte is manifested in the improvement of the general condition of patients with liver diseases, reduction of subjective complaints (such as weakness, a feeling of heaviness in the right hypochondrium, loss of appetite, itching of the skin, vomiting). Laboratory indicators improve: the activity of transaminases, gamma-glutamyltransferase, LF and the level of bilirubin in the blood plasma decrease. Pharmacokinetics. Absorption. After oral administration, silymarin is slowly and incompletely absorbed in the digestive tract. Subject to intestinal-hepatic circulation. Does not accumulate in the body.

Breeding. T _½ of silymarin is 6.3 hours. It is excreted mainly with bile in the form of glucuronides and sulfates.

Toxic liver damage: for maintenance therapy in patients with chronic inflammatory liver diseases and liver cirrhosis.

Capsules are taken orally, drinking a sufficient amount of liquid.

Adults and children over 12 years old. Treatment of severe liver damage begins with a dose of 1 capsule 3 times a day.

In cases of mild and moderate liver damage, take 1 capsule 1-2 times a day.

The course of treatment lasts at least 3 months.

Because of the possible estrogen-like effect of silymarin, it should be used with caution in patients with hormonal disorders (endometriosis, uterine fibroids, carcinoma of the breast, ovary and uterus, carcinoma of the prostate gland). In such cases, a doctor’s consultation is required.

The medicinal product contains wheat starch as an auxiliary substance. Wheat starch may contain gluten, but only in small amounts, and is therefore considered safe for celiac patients. Patients with a wheat allergy (which is different from celiac disease) should not take this medicine.

With the simultaneous use of silymarin and oral contraceptives and drugs used in estrogen therapy, a decrease in the effectiveness of the latter is possible.

Silymarin can enhance the effects of such drugs as diazepam, alprazolam, ketoconazole, lovastatin, atorvastatin, vinblastine, due to its inhibitory effect on the cytochrome P450 system.

By inhibiting the cytochrome P450 system, silymarin enhances the effect of antiallergic drugs (fexofenadine), anticoagulants (clopidogrel, warfarin).

Plant products containing silymarin are widely used as hepatoprotectors in oncological practice at the same time as cytostatics. Clinical studies show a slight risk of possible pharmacokinetic interactions of silymarin as an inhibitor of the isozyme CYP3A4 and UGT1A1 and cytostatics, which are substrates of these enzymes.

There are no reports of overdose. in case of accidental ingestion of high doses, vomiting should be induced, the stomach should be washed, activated charcoal should be taken and, if necessary, symptomatic treatment prescribed by the doctor should be used.
