Ascoril is a combined drug consisting of bromhexine, salbutamol and guaifenesin. Ascoril Extemporant syrup additionally contains menthol.

Active ingredients

Pathogenetically based treatment of cough – a common leading clinical symptom of acute and chronic respiratory diseases, is one of the urgent tasks of a medical specialist. Cough has various characteristics and is a frequent symptom of infectious and inflammatory pathology of the respiratory system. For symptomatic therapy, mucoactive drugs are used, containing either one active ingredient or a complex of various biologically active substances that affect several links in the pathogenesis of cough at once.

Salbutamol is a selective short-acting β2 agonist . Due to its presence in the composition of the drug, the following effects are achieved:

  • relaxes the smooth muscles of the bronchi and blood vessels;
  • prevents bronchospasm;
  • Stimulates the motor ability of the ciliated epithelium of the bronchi, increasing bronchial secretion and its evacuation.

Bromhexine is a mucolytic with secretolytic and secretokinetic effects. The main metabolite is ambroxol. The component provides:

  • decrease in sputum viscosity;
  • increases the volume of sputum and improves its discharge;
  • stimulation of endogenous surfactant synthesis;
  • improving the delivery of antibacterial drugs to lung tissue.

Guaifenesin – stimulates the secretion of the liquid part of bronchial mucus, reduces viscosity and prevents adhesion of sputum to bronchial tissues. Effects:

  • promotes the transition of cough from unproductive to productive;
  • increases the production of bronchial gland secretions;
  • facilitates coughing;
  • has an anti-anxiety and calming effect.

Menthol is a local irritant. It has been shown to have antimicrobial and anti-inflammatory properties, and also exhibits antispasmodic effects. It has a positive effect on the functions of the ciliated epithelium of the bronchi.

The well-chosen composition of Ascoril provides an effect on the leading mechanisms of cough pathogenesis in infectious inflammation of the tracheobronchial tree – mucostasis and bronchospasm. Thus, taking Ascoril has an expectorant, mucolytic and broncholytic effect.

Ascoril for adults is available in tablet form, for children over 2 years old – in the form of syrup.

Indications for use

Symptomatic treatment of productive cough in various respiratory diseases accompanied by bronchospasm.

Contraindication

Hypersensitivity to salbutamol, other sympathomimetics, bromhexine, guaifenesin, menthol or any of the other components of the drug. Coronary insufficiency, arrhythmia, other severe cardiovascular diseases, hyperthyroidism, severe liver dysfunction, gastric and duodenal ulcers.

Method of administration and doses

Adults and children over 12 years of age should take 1 tablet orally 3 times a day.

Children aged 6 to 12 years: ½-1 tablet 3 times a day.

The duration of treatment is determined individually.

Overdose

Symptoms: agitation, confusion, respiratory depression, rapid breathing, impaired consciousness, ataxia, diplopia, mild metabolic acidosis, arrhythmia, chest pain, hypotension up to shock, palpitations, tachycardia and severe tremor, especially in the hands. Gastrointestinal complaints, including nausea and vomiting, convulsions, extrasystole, drowsiness, headache, hyperglycemia, abdominal pain, exacerbation of gastric ulcer.

Treatment: Symptomatic therapy, electrocardiographic monitoring is indicated to monitor cardiac function.

Application features

The drug should be used with caution in patients suffering from glaucoma.

Very rarely, severe skin reactions, such as Stevens-Johnson syndrome and Lyell’s syndrome, occur with the use of bromhexine. If changes appear on the skin and mucous membranes, you should immediately consult a doctor and stop using the drug.

In case of impaired bronchial motility, accompanied by the formation of a large amount of bronchial secretion (malignant cilia syndrome), the drug should be used with caution due to possible stagnation of secretions.

In case of impaired renal function (including severe renal failure) or liver disease (with moderate and mild hepatic failure), it should be used with caution (reducing the dose or increasing the time interval between applications).

Periodic monitoring of liver function is recommended, especially with prolonged use.

Before starting treatment and during treatment of such patients, it is necessary to monitor blood glucose levels.

Isolated cases of myocardial ischemia have been reported in association with the use of salbutamol. Patients with cardiac disease (e.g., ischemic heart disease) treated with salbutamol sulfate should seek medical attention if they experience chest pain or other symptoms suggestive of worsening cardiac disease. Attention should be paid to evaluating symptoms such as dyspnea and chest pain, which may be due to both cardiac and respiratory disease.

Treatment with β2 -agonists may result in severe hypokalemia, and monitoring of serum potassium is recommended.

Like other beta-blocking agonists, salbutamol sulfate may cause reversible metabolic changes, such as increased blood sugar levels. As a result, there have been isolated reports of ketoacidosis in diabetic patients. Concomitant use of corticosteroids may exacerbate this condition.

Do not administer to patients before anesthesia.

The drug should not be used in patients with hypertrophic cardiomyopathy.

If there is a history of gastric bleeding, the drug should be used under the supervision of a physician. During treatment with the drug, it is necessary to drink a sufficient amount of fluid, which increases the expectorant effect of bromhexine.

Special safety precautions

Salbutamol, like other stimulant β-blockers, should be prescribed with caution to patients with seizure disorders and diabetes mellitus.

In some patients, as with any other β-blocker agonist, clinically significant changes in systolic and diastolic blood pressure may occur. It is known that β-blocker agonists can cause ECG changes such as T wave flattening, QT prolongation, and ST segment depression. Therefore, salbutamol should be used with caution in patients with cardiovascular disease, especially in patients with hypertension.

The need to increase the dose of salbutamol indicates worsening asthma control and requires a review of therapy, if necessary, the appointment of corticosteroids.

Guaifenesin should be used with caution in the treatment of cough with excessive sputum production, persistent or chronic cough resulting from smoking, asthma, chronic bronchitis, emphysema.

Composition

Active ingredients in one tablet:

  • salbutamol sulfate, equivalent to salbutamol – 2 mg
  • bromhexine hydrochloride – 8 mg
  • guaifenesin – 100 mg

Excipients: calcium hydrogen phosphate, corn starch, methyl parahydroxybenzoate (E 218), propyl parahydroxybenzoate (E 216), talc, colloidal anhydrous silicon dioxide, magnesium stearate.

Storage conditions

Store in a dry place, protected from light, at a temperature not exceeding 25 °C.
