Pimafucin is an antifungal agent. The active substance of the drug is natamycin, a macrolide antibiotic of the polyene series. It has a broad spectrum of action. It exhibits fungicidal activity, since it blocks the sterols of the cell membranes of fungi, which leads to the death of the microorganism. It is sensitive to fungi of the genus Candida spp. It exhibits somewhat less sensitivity to dermatophytes. To date, no cases of resistance to natamycin have been observed in clinical practice.

Pharmacological properties

Pharmacodynamics. Natamycin is a broad-spectrum polyene antibiotic, active against pathogenic fungi, including yeasts and especially Candida albicans, which are most often the cause of genital tract infections. It acts fungicidally, binding sterols of cell membranes, disrupting their integrity, causing the death of microorganisms.

The drug does not exhibit sensitizing activity. No cases of allergy to natamycin have been identified during treatment or during its industrial production, and no development of resistance to it has been noted.

Thanks to the enteric coating, the tablet acts only in the intestines.

The drug acts quickly and effectively in case of infection with vaginal yeast fungi (candidiasis). Vaginal suppositories also have a therapeutic effect in the presence of vaginal discharge, inflammation and itching, ending with the upper, less accessible parts of the vagina. The drug quickly melts in the vagina, forming a foamy mass, which contributes to the uniform and complete distribution of the active substance over its entire surface.

Pharmacokinetics. When taken orally in tablet form, natamycin is practically not absorbed in the gastrointestinal tract.

The currently existing methods failed to detect natamycin, which was applied intravaginally, and its derivatives in the blood and biological fluids of the body, which indicates the absence of systemic action of the drug.

Natamycin is active when applied topically and is not absorbed through the skin and mucous membranes.

Indication

Tablets: fungal intestinal diseases; vaginal suppositories: vaginitis caused by candida albicans; cream: fungal skin lesions caused by candida albicans.

Application

 

Tablets. For the treatment of intestinal candidiasis, adults are prescribed 1 tablet 4 times a day. Usually, therapy lasts 1 week.

Children aged 3 years and older are prescribed 1 tablet 2 times a day. Given the peculiarities of the pharmacodynamic and pharmacokinetic properties of natamycin, the same dose is recommended for children of different ages. Usually the duration of treatment is 1 week.

Suppositories. Used in adult women 1 suppository for 3-6 days. Suppositories are recommended to be inserted as deeply as possible into the vagina (in the supine position) once a day (at night). If treatment is necessary for several days, the course of therapy should be completed before the onset of menstruation or treatment should be started after it has ended.

The cream is applied to the affected areas of the skin 1-4 times a day daily. The duration of therapy is usually 2-3 weeks.

Contraindication

Hypersensitivity to the components of the drug.

On the part of the gastrointestinal tract: nausea and diarrhea are possible (which occur in the first days of use and disappear on their own during treatment).

Hypersensitivity reactions: Some irritation or burning sensation may occur at the site of application. If they persist, this may be due to hypersensitivity to the preservatives (parahydroxybenzoates). The presence of methylparahydroxybenzoate and propylparahydroxybenzoate in the preparation may cause allergic reactions (possibly delayed).

Special instructions

The use of natamycin in tablet form may be associated with the development of an acute attack of porphyria, therefore, the use of the drug is considered dangerous for patients with this disease.

Since the drug in tablet form contains lactose, it should not be used in patients with rare hereditary forms of galactose intolerance, the Lapp lactase deficiency or glucose-galactose malabsorption syndrome.

The drug contains sucrose, which should be taken into account by patients with diabetes mellitus. Since the drug contains sucrose, it should not be used in patients with rare hereditary diseases such as fructose intolerance or sucrase-isomaltase insufficiency.

During menstruation, treatment with Pimafucin ® in the form of vaginal suppositories should be interrupted. There is no need to avoid sexual contact during therapy with the drug.

Pregnancy and breastfeeding. Pimafucin ® can be prescribed during pregnancy and breastfeeding.

Children. Natamycin in the form of film-coated tablets is used in children over 3 years of age; in the form of cream – in children of all age groups; in the form of vaginal suppositories – is not used in children.

Overdose

There are no reports. When using vaginal suppositories, systemic effects are practically excluded, since the drug is not absorbed through the mucous membrane.
