Relief Advance ointment is mainly used for external hemorrhoids accompanied by severe pain and/or burning. Contains local anesthetic benzocaine. Benzocaine helps to get rid of heartburn and pain in the anorectal region during exacerbation of hemorrhoids, after proctological operations and diagnostic manipulations.
Composition
1 g of ointment contains:
Active substance: benzocaine – 200 mg.
Excipients: light mineral oil, soft white paraffin, propylene glycol, methylparaben (E 218), propylparaben (E 216), sorbitan stearate.
Pharmacological properties
Pharmacological
Benzocaine back stabilizes the membrane of the neuron, which reduces its permeability to sodium ions. Depolarization of the neuron membrane is suppressed, and thus blocks the occurrence and conduction of nerve impulses. Thus, benzocaine has a pronounced local analgesic effect.
Pharmacokinetics
Drug for local use. Benzocaine, which is part of it, is excreted in mucus or, in extremely small amounts, by the kidneys.
Indication
External and internal hemorrhoids, erosions, cracks, diseases of the rectum, accompanied by severe pain, itching, burning in the anorectal region; in the postoperative period in patients who underwent proctological operations; for pain relief during diagnostic manipulations.
Contraindication
Hypersensitivity to the components of the drug, including excipients, and other amide local anesthetics, thromboembolic disease, granulocytopenia.
Method of application and dosage
Adults and children over 12 years of age should use the drug after performing hygienic procedures. Remove the protective cap from the applicator. Attach the applicator to the tube and squeeze out a small amount of ointment to lubricate the applicator. The ointment is carefully applied through the applicator to the affected areas outside or inside the anus up to 4 times a day, especially in the evening, in the morning or after each bowel movement for 7-10 days.
After each use, rinse the applicator thoroughly and place it in the protective cap.
Do not use if protective coating is missing or damaged.
Overdose
Overdose symptoms may be related to the pharmacological effects of benzocaine. Its systemic absorption in case of an overdose can manifest itself in drowsiness, anxiety, heart rhythm disturbances, disturbances. In severe cases – convulsions, coma, decreased breathing rate or respiratory failure. There are reports that prescribing products containing benzocaine can cause methemoglobinemia. Symptoms such as cyanosis of the skin, lips and nail beds, headache, dizziness, shortness of breath (difficulty breathing), weakness, tachycardia, may be observed during treatment, may indicate methemoglobinemia, which is potentially life-threatening and requires urgent medical intervention.
Emergency aid for overdose and the development of methemoglobinemia consists in intravenous administration of methylene blue. Other symptoms of overdose require withdrawal of the drug and symptomatic treatment.
Adverse reactions
From the blood and lymphatic system. When using the drug, methemoglobinemia is possible (cyanosis of the skin, lips and nail beds, headache, dizziness, shortness of breath, weakness, tachycardia).
General disorders and changes at the site of application: allergic reactions, including hyperemia, skin rashes, itching, pain at the site of application, changes at the site of injection (irritation, swelling).
From the side of the skin and subcutaneous tissue: allergic dermatitis, contact dermatitis.
Features of application
Use during pregnancy or breastfeeding
During pregnancy, the drug should be used only in case of extreme necessity, after a careful assessment by the doctor of the ratio benefit for the mother / risk for the fetus or child.
During the period of breastfeeding, a decision should be made to stop breastfeeding or refuse to use the drug, taking into account the importance of taking the drug for the mother.
Children
The effectiveness and safety of the drug Relief Advance in children under 12 years of age has not been studied. There are no data.

Interaction with other medicinal products and other types of interactions
It is not recommended without strict medical indications and doctor’s supervision to use the drug against the background of regular intake of MAO inhibitors (MAO), antidepressants and hypotensive agents due to the theoretical possibility of reducing the effect of MAO inhibitors or enhancing the effect of hypotensive agents as a result of interaction with benzocaine in cases of significantly exceeding the recommended doses and duration of administration .
With local use of the drug Relief Advance, rectal ointment in the anus area, auxiliary substances white soft paraffin can reduce the strength and safety of latex condoms, which are used simultaneously with the drug (in order to prevent pregnancy and / or reduce the likelihood of infection with transmitted diseases sexually transmitted or human immunodeficiency virus).
