Pharmacological properties

Pharmacodynamics. phenylephrine hydrochloride is an adrenomimetic, the use of which leads to a local vasoconstrictor effect. it normalizes the ratio between blood flow to hemorrhoidal nodes and blood outflow from them, and also helps to reduce exudation, swelling, itching, serous secretions in hemorrhoids and other diseases of the anorectal region.

Pharmacokinetics. Relief – a preparation for local use. Phenylephrine, included in its composition, is inactivated by tissue MAO, inactive metabolites are excreted in mucus or in small amounts by the kidneys.

Indication

Relief, ointment. symptomatic treatment of itching, burning and discomfort accompanying hemorrhoids and other diseases of the anorectal zone, as well as erosions, cracks, microtraumas in the anus area.

Relief suppositories. External and internal hemorrhoids, fissures of the anus, erosion and microtraumas of the anus, anal itching.

Application

In adults and children aged 12 and over, the drug should be used after performing hygienic procedures.

Relief ointment. Remove the protective cap from the applicator. Attach the applicator to the tube and squeeze out enough ointment to lubricate the applicator. Apply the ointment carefully through the applicator to the affected areas outside or inside the anus in an amount sufficient to treat the affected areas up to 4 times a day, preferably in the evening, in the morning or after each bowel movement. Apply until the symptoms of the disease disappear. After each use, rinse the applicator thoroughly and place it in the protective cap. Do not use if protective coating is missing or damaged.

Relief suppositories. First, wash the skin around the anus with warm water, clean the affected area with a moistened soft napkin, then carefully dry it with toilet paper or a soft cloth, removing excess moisture.

Before inserting the suppository, it is necessary to remove the plastic protective cover from it. Insert the suppository into the anus.

Enter 1 suppository up to 4 times a day, especially at night, in the morning and after each bowel movement. The duration of treatment is 7-14 days. Regular use of the drug allows you to ensure a stable therapeutic effect, relieves the symptoms of hemorrhoids. Do not use if protective coating is missing or damaged.

Children. The effectiveness and safety of the use of the drug Relief in children under 12 years of age has not been studied. There are no data.

Contraindication

Hypersensitivity to any component of the drug, a significant increase in heart rate, tachysystolic heart rhythm disorders, decompensated heart failure, cardiac conduction disorders, severe renal and hepatic failure, thyrotoxicosis, acute pancreatitis, simultaneous use with MAO inhibitors or within 15 days after stopping treatment with inhibitors mao, thromboembolic disease, granulocytopenia.

Side effects

The drug is usually well tolerated, but in some cases side reactions due to phenylephrine hydrochloride are possible.

From the side of the cardiovascular system: reflex bradycardia, arrhythmia, possible heart rhythm disturbances, increased blood pressure.

From the endocrine system: increasing symptoms of hyperthyroidism.

From the side of the immune system: allergic reactions, including cross-reactions with allergies to other sympathomimetics, which may manifest as rashes, itching, angioedema.

From the side of the nervous system: dizziness, nervousness, headache, tremor, insomnia, feeling of fear, feeling of blood flow to the face.

At the site of application, irritation, hyperemia, rash, itching, swelling, moderate pain / burning in case of damage or bleeding from the anus are possible.

Methyl parahydroxybenzoate and propyl parahydroxybenzoate can cause allergic reactions (perhaps delayed) and in some cases – bronchospasm. Lanolin can cause local skin reactions (e.g. contact dermatitis). For Relief, ointment: benzoic acid moderately irritates the skin, eyes and mucous membrane.

Special instructions

In case of significant bloody secretions from the anus or in the presence of symptoms of the disease for more than 7 days of treatment, or in case of worsening of the condition, it is necessary to consult a proctologist.

The drug should be used with caution in patients with hypertension, cardiovascular or cerebrovascular diseases, increased thyroid function (phenylephrine can cause adverse cardiovascular effects, especially when using high doses or in sensitive individuals), glaucoma.

Patients with diabetes should be careful when using this drug: phenylephrine can increase the level of glucose in the blood. These effects are usually temporary and mild and may be exacerbated by exceeding recommended doses. The level of glucose in the blood should be carefully monitored.

Persons with urinary disorders due to hypertrophy or the presence of neoplasms of the prostate gland: phenylephrine can worsen urination in patients with hypertrophy of the prostate gland due to spasm of the smooth muscles of the bladder neck, which occurs as a result of stimulation of α 1 -adrenoceptors.

With hepatitis, urinary disorders, the use of the drug is permissible, taking into account the benefit / risk ratio, which is determined by the doctor.

In the case of reaching the appropriate level of systemic absorption, the possibility of interaction of phenylephrine, applied locally in the anorectal region, with MAO inhibitors, which leads to potentiation of the hypertensive effect, should be taken into account.

It should be considered the possibility that phenylephrine, applied locally in the anorectal region, can cause severe hypertension due to stimulation of α-adrenoceptors in some individuals when this agent is used simultaneously with antihypertensive drugs.

To minimize systemic effects, the recommended dose should not be exceeded, unless otherwise recommended by a doctor.

The drug in the form of suppositories should not be used in cases of damage or absence of a protective plastic shell.

Use during pregnancy and breastfeeding. Since the safety of the use of the drug Relief (phenylephrine) during pregnancy has not been studied, its use is possible if, in the opinion of the doctor, the benefit to the mother exceeds the potential risk to the fetus / child.

Pregnant women should consult a doctor before using the drug.

Because it is not known whether topical application of Relief (phenylephrine) can lead to sufficient systemic absorption to cause measurable excretion of phenylephrine in breast milk, the physician should determine the importance of the drug to the mother: whether it is necessary to discontinue breastfeeding or discontinue the drug.

Studies on the effect of the medicinal product on fertility have not been conducted.

Interactions

The drug should not be used simultaneously with other vasoconstrictors (with any route of administration of the latter), as well as with antihypertensive drugs (β-adrenoceptor blockers), since phenylephrine can cause severe ag in some people due to stimulation of α-adrenoceptors.

Atropine sulfate blocks the reflex bradycardia caused by phenylephrine and increases the vasopressor response to phenylephrine. Simultaneous administration of phenylephrine with β-adrenoceptor blockers can lead to hypertension and excessive bradycardia with possible heart block. It should be used with caution with thyroid hormones, drugs affecting cardiac conduction (cardiac glycosides, antiarrhythmic drugs).

With simultaneous use with drugs that contribute to the excretion of potassium, for example, with some diuretics such as furosemide, it is possible to increase hypokalemia and reduce arterial sensitivity to such vasopressor drugs as phenylephrine.

The simultaneous use of phenylephrine and other sympathomimetics can lead to additional stimulation of the central nervous system to an extremely high level, which is accompanied by nervousness, irritability, insomnia. Seizures are also likely.

When using MAO inhibitors, you should consult a doctor before starting the use of phenylephrine in the anorectal region. MAO inhibitors used before the start of treatment with phenylephrine potentiate the cardiac and pressor effects of the latter, since the metabolism of phenylephrine is reduced.

With local use of the drug Relief rectal ointment, in the anus, the auxiliary substance white soft paraffin can reduce the strength and safety of latex condoms, which are used simultaneously with the drug.

Overdose
