Pharmacological properties

Selenium is one of the most important trace elements. In human blood plasma, selenium binds to glutathione peroxidase and a selenium-transporting protein known as selenoprotein P, in the form of the amino acid selenocysteine. Selenium-containing glutathione peroxidase is a component of the antioxidant defense system of cells.

There is a known inverse relationship between the level of selenium in the blood plasma and the likelihood of developing diseases of the cardiovascular system (cardiomyopathy, arteriosclerosis, myocardial infarction), as well as oncological diseases. In patients with renal failure and gastrointestinal diseases, a decrease in the level of selenium in the blood plasma is noted.

When taken orally, sodium selenite is absorbed mainly in the duodenum. In the blood, selenium is absorbed by erythrocytes and reduced by enzymes to selenide. Selenide is the central reservoir of selenium for excretion from the body and for the formation of complexes with selenoproteins. Excess selenide is metabolized by methylselenol and dimethylselenide into trimethylselenium ions – the main excretion product. The total amount of selenium in the human body is 4-20 mg. The excretion of selenium from the human body depends on the dose consumed and the level of selenium in the body. Selenium is excreted with feces, urine, and also through the lungs in the form of trimethylselenium ions and other metabolites.

Indication

Selenium deficiency in the body has been established, occurring in cases of digestive disorders/malabsorption and malnutrition (e.g. total parenteral nutrition), which cannot be compensated for through diet.

Application

Tablets should be taken whole, without chewing, with a small amount of liquid, after meals. Unless otherwise prescribed, the dose is 100 mcg of selenium per day. This dose can be increased for a short period to 300 mcg of selenium. Treatment is carried out until the normal level of selenium in blood plasma is restored – 80-120 ng / ml, in whole blood – 100-140 ng / ml.

Contraindication

Hypersensitivity to the components of the drug, intoxication due to selenium poisoning.

Side effects

Hypersensitivity reactions are possible.

Special instructions

Patients with rare hereditary problems of fructose intolerance, glucose-galactose malabsorption or sucrase-isomaltase insufficiency should not take this medicine.

If you miss a dose, you should continue taking the drug as recommended and do not take a double dose to make up for the missed one. Even after prolonged use of the drug, addiction and withdrawal syndrome do not develop.

Pregnancy and breastfeeding. Provided that the drug is taken as directed, there are no restrictions on use during pregnancy and breastfeeding.

Children. Selenium is not contraindicated in children, but this dosage form is not used in children.

Does not affect the reaction speed when driving or working with other mechanisms.

Interactions

It is necessary to avoid taking cefacel simultaneously with ascorbic acid, as this leads to a decrease in the bioavailability of selenium.

Overdose

Symptoms of acute overdose may include a garlicky breath odor, fatigue, nausea, diarrhea, and abdominal pain. In case of chronic overdose, nail and hair growth disorders, as well as peripheral polyneuropathies, are noted. In cases of acute overdose, gastric lavage and symptomatic therapy should be performed. In cases of chronic overdose, the dose of the drug should be reduced.
