Pharmacological properties

Vibrocil is a combination drug containing phenylephrine and dimethindene.

The drug reduces nasal discharge and helps cleanse the nasal passages without disrupting the physiological functions of the ciliated epithelium and nasal mucosa.

Phenylephrine belongs to sympathomimetic amines. It is used as a nasal decongestant with a moderate vasoconstrictor effect, selectively stimulating α 1 adrenergic receptors of the cavernous venous tissue of the nasal mucosa. Thus, it quickly and permanently eliminates swelling of the nasal mucosa and its paranasal sinuses.

Dimetindene – an antagonist of histamine H1 receptors – has an antiallergic effect, is effective when used in low doses, and is well tolerated.

Vibrocil is applied topically, so its activity does not correlate with the concentration of active substances in blood plasma.

With accidental oral absorption, the bioavailability of phenylephrine decreased and was about 38%, T ½ – about 2.5 hours.

The systemic bioavailability of dimethindene after oral administration in solution is about 70%, T ½ – about 6 hours.

Indication

Symptomatic treatment of colds, nasal congestion, acute and chronic rhinitis, seasonal (hay fever) and year-round allergic rhinitis, acute and chronic sinusitis, vasomotor rhinitis. Adjunctive therapy for acute otitis media.

Preparation for surgical intervention in the nasal area and elimination of swelling of the nasal mucosa and paranasal sinuses after surgical intervention.

Vibrocil nasal gel is indicated for dry nasal mucosa, crusting due to nasal trauma, and to prevent nasal congestion at night during sleep.

Application

Before administering the drug, the nose should be thoroughly cleaned.

Nasal drops. Children under 1 year of age (on the recommendation of a doctor): 1 drop in each nostril 3-4 times a day.

Children aged 1 to 6 years: 1-2 drops in each nostril 3-4 times a day.

Adults and children over 6 years of age: 3-4 drops in each nostril 3-4 times a day.

Nasal gel. Adults and children over 6 years of age: instill a small amount of gel as deeply as possible into each nasal passage 3-4 times a day. It is recommended that the last application of the gel be done before bedtime.

Nasal spray. Adults and children over 6 years of age: 1-2 sprays into each nostril 3-4 times a day. The bottle should be held vertically, with the sprayer pointing upwards. Keeping the head straight, insert the tip of the bottle into the nostril, squeeze the sprayer once with a short, sharp movement and, after pulling it out of the nose, release it. During the injection, it is recommended to breathe in lightly through the nose.

The duration of treatment should not exceed 7 days and depends on the course of the disease.

Contraindication

Hypersensitivity to any component of the drug; due to the phenylephrine content, the drug is contraindicated in atrophic rhinitis, as well as in patients who are taking MAO inhibitors or have taken them in the previous 14 days.

The drug in the form of a gel and spray is not recommended for children under 6 years of age. It is not recommended to take Vibrocil during pregnancy and breastfeeding.

Side effects

The drug is usually well tolerated. In some cases, mild and transient local reactions on the part of the nasal mucosa (burning sensation or dryness) are possible. Very rarely: the development of allergic reactions (in particular, local skin reactions, itching of the body, swelling of the eyelids, face, general weakness).

Special instructions

Vibrocil should not be used for longer than 1 week. Prolonged or excessive use of the drug may cause tachyphylaxis and rebound effect (rhinitis medicamentosa).

As with other vasoconstrictors, the recommended dose should not be exceeded. Excessive use of Vibrocil, especially in children and the elderly, may cause systemic effects of the drug.

The drug should be prescribed with caution to patients with cardiovascular diseases, hypertension, thyroid diseases, and glaucoma.

Pregnancy and breastfeeding. The drug is not prescribed.

Children. Vibrocil in the form of drops is used in children under 1 year of age only as prescribed by a doctor. Vibrocil in the form of spray and gel are not used in children under 6 years of age.

Impact on the ability to drive vehicles and operate machinery. No effect.

Interactions

The drug is contraindicated in patients who are taking MAO inhibitors or have taken them in the previous 14 days. The gel is contraindicated in atrophic rhinitis.

Vasoconstrictors should be prescribed with caution to patients taking tricyclic antidepressants and antihypertensive drugs such as β-adrenergic blockers.

Overdose

No serious side effects have been reported in cases of accidental oral administration of the drug by young children.

Most cases were asymptomatic; fatigue, stomach pain, mild tachycardia, increased blood pressure, agitation, insomnia, and pale skin have been reported very rarely.

Treatment: use of activated charcoal, possibly laxatives in young children (gastric lavage is not required); adults and older children are prescribed plenty of fluids to drink.

Storage conditions

In a place protected from light and heat; drops, gel – at a temperature not exceeding 30 °C.

Spray – at a temperature not exceeding 25 °C.
