Pharmacological properties
Pharmacodynamic parameters. Nitrobenzene derivative nimesulide is the active pharmaceutical ingredient of Nimid. Nimesulide inhibits the COX-mediated process of converting arachidonic acid into a precursor of prostaglandins, prostacyclin and thromboxane A2. Nimesulide exhibits both analgesic, antipyretic and anti-inflammatory effects mediated by the action of COX-2, but has little effect on platelet function or the loss of gastric cytoprotection associated with COX-1 activity.
The therapeutic effect of nimesulide is the result of a complex of its effects aimed at inhibiting a number of key mediators of the inflammatory process, such as prostaglandins, free radicals, proteolytic enzymes and histamine, tumor necrosis factor α, as well as platelet activation factor.
Topical application of the drug leads to the disappearance or reduction of the intensity of pain (arthralgia, among other things) during movement and at rest, reduces morning stiffness and swelling in the joints, and helps to expand the range of motion in the joints.
At the early stage of the inflammatory process, nimesulide is not inferior in anti-inflammatory effect to piroxicam and indomethacin in equimolar concentration.
Pharmacokinetic parameters. Topical application of nimesulide ensures its gradual percutaneous penetration into the underlying tissues and synovial fluid. It almost does not enter the systemic circulation, therefore no significant systemic effect is noted.
Indication
Topical therapy for diseases of the musculoskeletal system (tendosynovitis, post-traumatic tendonitis, periarthritis, osteoarthritis, muscle strains, heavy physical exertion on the joints), which are manifested by stiffness of movements, pain and inflammatory process.
Application
Adults externally 3-4 times a day. Before the procedure, wash and dry the painful area of ​​skin, then spread a thin layer of gel (3 cm) on it, rubbing it in slightly. Duration of treatment ≤4 weeks, depends on the individual effectiveness of the therapy.
Contraindication
- Hypersensitivity to the active substance of the drug or to any of its components; skin infectious diseases; dermatitis; impaired skin integrity; allergic reactions (urticaria, bronchospasm, rhinitis) to prostaglandin synthesis inhibitors in history.
Side effects
Mild to moderate local skin irritation (allergic reactions, limited hyperemia, itching, rash, peeling).
Vasomotor rhinitis, angioedema, shortness of breath, bronchospasm (rare in sensitive individuals).
Special instructions
Monitoring of the condition of elderly patients with congestive heart failure, impaired hepatic or renal function, as well as patients with severe pathology of the coagulation system, hemorrhages or ulcers of the upper digestive tract is required.
Concomitant use with other topical medications is not recommended.
Do not apply to areas of broken skin or under occlusive dressings.
Avoid contact with mucous membranes and eyes.
Use for the shortest period of time at the lowest dose necessary to control symptoms.
In the absence of positive dynamics, it is necessary to consult a doctor.
Do not use in case of hypersensitivity to nonsteroidal anti-inflammatory drugs. If this condition occurs, therapy is discontinued.
Given the likelihood of photosensitization when using the product, it is necessary to refrain from visiting tanning salons and UV radiation.
Do not prescribe during pregnancy and breastfeeding.
There is no effect on the reaction speed when driving vehicles or other mechanisms.
There are no data on the efficacy and safety of use in children.
Interactions
Not detected (in case of external use).
However, it is necessary to take into account the possibility of nimesulide entering the systemic circulation. In this case, due to the displacement of some drugs from plasma proteins, their effect and toxicity may be enhanced. Thus, caution is required when used simultaneously with other NSAIDs, methotrexate, cyclosporine, digoxin, antihypertensives, diuretics, anticoagulants, phenytoin, lithium preparations, oral hypoglycemic drugs.
With simultaneous topical use of several nonsteroidal anti-inflammatory drugs, local irritation (peeling, hyperemia, urticaria) may occur.
The anti-inflammatory effect is enhanced when used simultaneously with corticosteroids, aminoquinolones, and gold preparations.
Overdose
When using the gel on a large area of ​​skin or if the specified dose is exceeded, there is a possibility of developing systemic side effects typical of NSAIDs (such as cephalalgia, dizziness, dyspepsia, epigastralgia).
The dose is reduced or the drug is discontinued. Treatment is symptomatic.
Storage conditions
